Job duties:
You will serve as the Diabetes Program Coordinator for the Chronic Disease Prevention Program. The Diabetes Coordinator manages the diabetes-related work of the Program’s U.S. CDC grant, “Improving the Health of Americans Through Prevention and Management of Diabetes and Heart Disease and Stroke CDC-RFA-DP18-1815.” This grant is designed to increase health plan coverage for National Diabetes Prevention Program, improve clinical workflows for the identification of prediabetes, and to promote referrals to and participation in evidence-based programs such as Diabetes Self-Management Education and Support and National Diabetes Prevention Program.
You will also:
• Provide subject matter expertise in diabetes prevention and self-management.
• Provide technical assistance to health care providers and evidence-based program providers to ensure efficient operation of diabetes prevention and self-management programs.
• Represent Maine CDC in cooperation with clinicians, health systems, community-based organizations, and diabetes stakeholder groups.
• Lead diabetes-related Quality Improvement and Practice Facilitation efforts in primary care practices and health systems.
• Review and maintain up-to-date knowledge of diabetes prevention, self-management, and treatment best and emerging practices.
• Maintain the Professional Diabetes Educator Program training curriculum to ensure a competent diabetes educator workforce in Maine.
• Assist the Chronic Disease Prevention Program in implementing a comprehensive, collaborative approach to addressing chronic diseases in Maine.
MINIMUM QUALIFICATIONS:
A Bachelor’s Degree in Public / Community Health, Health Administration, Health Sciences or Health Education; Public Administration, Government, Social Work, or a closely related field and three (3) years professional experience in the planning, development, coordination and/or evaluation of health programs or services -OR- an equivalent seven (7) year combination of education, training, and experience.
Preferred candidates will have:
• Excellent oral and written communication skills.
• Project Management skills and the ability to plan for and work on multiple tasks simultaneously.
• Knowledge and experience with evidence-based diabetes prevention and self-management program operations and management.
• Knowledge of and experience with U.S. CDC grant management requirements.
• Knowledge of and experience with implementing systems change, Practice Facilitation, Quality Improvement, and adapting clinical workflows.
• Experience convening and collaborating with various stakeholders, in both public and private sectors.
• Ability to understand and act upon surveillance and evaluation data to improve Program operations.
