Job Summary
- Under minimal supervision, provides telephonic coaching, counseling, consultation, information and referral services to high risk members of Denver Health Medical Plan who require guidance in managing various chronic health conditions.
- Encourages members to actively participate and effectively manage their care through an interactive process of assessment, education, and collaborative problem-solving.
- Assesses and documents results, goals, interventions and outcomes into a web-based care management software system. Responsible for facilitating groups and/or conducting seminars focused on health and wellness prevention.
- Works closely and collaboratively with an interdisciplinary care management team to outreach, engage and provide members with the information and skills to facilitate health behavior change.
Minimum Qualifications
Education: Master’s degree in Clinical Psychology, Social Work, Public Health, Education, Nutrition, or Exercise Physiology, or, a Bachelor’s degree in one of the above fields plus a minimum of five years of directly related experience in health coaching, health education or health promotion.
Experience: Typically, three years of experience with diverse populations in a health care setting such as hospitals, clinics, or home health agencies, in providing patient education and counseling as a main intervention to chronic disease management and health/wellness promotion.
Knowledge, Skills & Abilities:
- Knowledge of specific disease and lifestyle related topics including, but not limited to, smoking cessation, weight management, nutrition, stress reduction and other chronic health conditions.
- Skilled at assessment and able to provide a variety of clinical and coaching services, techniques, and/or models that match the member’s readiness to change.
- Able to communicate effectively and work with a wide range of health care professionals to develop a care plan.
- Knowledge of health and wellness education regarding preventative care measures.
- Must be able to work independently and have excellent problem-solving abilities.
- Bilingual in Spanish is highly preferred.
Certificate/License/Registration: All applicable Certifications and Licenses required for this job must be kept current as a condition of continued employment.
