Description: Community Care Self Management--Program Description: We provide coordination, observations, referrals, advocacy and monitoring of children with Axis I diagnosis, for parents or guardians. Population served: birth to 20 children with developmental or emotional disabilities. Program Mission: Woodfords Family Services is committed to the support and inclusion of individuals with special needs and their families in Maine communities.
