Description & Responsibilities:
Mount Prospect Academy - New Hampshire Community Based Services offers different community based programs: Project Connect In-Home Individual Service Option, Solid Foundations Home Based Therapeutic Services, and FAST Forward NFI. These programs operate with the goal of preventing and/or minimizing out-of-home placements and helping youth transition back to their home communities from residential care. With the right combination of supports, many youth, regardless of their presenting concerns, can remain safely in their natural environment without long term residential placement.
MPA - NH Community Based Services provides individual counseling, family counseling, group counseling, educational counseling, case management and service coordination, psychoeducation and parent training, substance abuse counseling and education, anger and behavioral management, trauma informed care, preparation for independent living, role modeling and applied learning in the community, pro-social community based groups, GROW curriculum, ARC Framework, trauma based treatment, Trauma Focused Cognitive Behavioral Therapy, Adventure therapy groups for latency and adolescent aged youth, and NH SAU additional service options for students.
MPA’s Community Based Services are intended to provide short term support and stabilization. As such, services provided by the PS intern and their teams should generally be focused on helping clients and their families identify and utilize long-term community supports and natural supports as well as agencies in the community. Within that framework, the PS intern role is to provide overall leadership on the case in terms of permanency and family and youth guided treatment. The PS intern is responsible for establishing clear treatment goals and clinically appropriate recommendations for treatment and interventions. The PS intern directs the activities of other team members (EG. Permanency Coaches) to ensure client and family interactions are goal oriented and clinically appropriate. The PS intern will provide case management and clinical (individual, family, group) support to children and their families.
The PS intern will perform some or all of the following services: provide clinical support for individuals and their families to overcome obstacles, identify strengths and develop support networks that will help them overcome barriers; provide support, education and guidance to assist in developing structure, routines, effective parenting skills, positive communication, safety and behavioral plans, as well as hygiene plans. Other services provided are based on individual and family needs (i.e., nutrition and meal
planning, household budgeting, etc.)
Responsibilities:
• Complete tasks associated with client intake process
• Create and update treatment plans including student and caregiver treatment goals
• Provide individual and family therapy as indicated in the treatment plan
• Provide case management, including but not limited to employment outreach, housing
outreach, acquisition of household goods and other resources, community service coordination
• Collaborate with referring contacts and other case collaterals including therapists, guidance
counselors, physicians, psychiatrists, probation officers and attorneys.
• Ensure proper filing of all documentation using Extended Reach medical records system protocol.
• Prepare for periodic treatment review meetings by completing progress reports and updated treatment plans.
• Support, coordinate, and co-facilitate groups, including prosocial recreational activities
• Provide transportation to clients and families in appropriate situations according to treatment plan goals, ie. to attend group or register for food stamps
• Attend regular office meetings, including weekly faculty meetings and case reviews
• Shadow Permanency Specialists and Permanency Coaches in various meetings, including family therapy sessions, school meetings, treatment review meetings, and case reviews
• Complete and file documentation in accordance with MPA timelines, including SOAP notes,
monthly reports, and weekly reports
• Attend supervision as requested by supervisor, at a minimum of weekly
• Complete other miscellaneous tasks to support overall functioning of designated office as
determined by supervisor
Qualifications:
• Master’s degree in progress in social work, human services, psychology or related field
• Valid driver’s license, insured transportation, ability to transport clients in personal vehicle. Position requires satisfactory completion of background and other record(s) checks.
Operates Monday through Friday 9:00am to 5:00pm

Additional Qualifications:
Holding a valid driver's license and insured vehicle. Ability to drive and transport clients in the community for individual counseling in the community. Mondays are required to be at placement to participate in Team Meeting and Peer Supervision. Work independently and collaboratively within a team environment, flexibility and adaptability, excellent communication, consultation with clinical supervisor, and the ability to start learning opportunities quickly within the first month.
